By selecting my drop off time, I am stating that I will be home during that time period. If I miss my delivery time, my invoice will be credited back to my account and a 10% restocking fee will apply – unless other arrangements have been made to drop off the product as indicated below. If I am not able to be home, I consent to leaving my product in the area listed below (front deck, back door, reception area at work…). By listing a drop off area, I understand that 24 HR Animal Care Centre will not be found liable for any missing product.

I have a valid Delivery Authorization Form set up with 24 HR Animal Care Centre and authorize the above stated charges to be applied to my credit card. If I have not set up a Delivery Authorization Form, this form must first be filled out in person at 24 HR Animal Care Centre.
